Dear Patients and Families,

In light of the current situation with trying to contain the spread of the coronavirus, please take all the necessary precautions to make your visit safe in the midst of this health crisis:

Follow the guidance outlined below:
  • Stay at home if you begin to feel unwell, even with mild symptoms such as headache and slight runny nose, until you recover. Why? Avoiding contact with others and visits to medical facilities will allow these facilities to operate more effectively and help protect you and others from possible COVID-19 and other viruses.
  • Reschedule your appointment for when your symptoms have subsided
  • Advise our office if you have recently traveled abroad (within the last 2 weeks)
  • If you develop fever, cough and difficulty breathing, seek medical advice promptly as this may be due to a respiratory infection or other serious condition. Call in advance and tell your provider of any recent travel or contact with travelers. Why? Calling in advance will allow your health care provider to quickly direct you to the right health facility. This will also help to prevent possible spread of COVID-19 and other viruses.

We ask you refrain from wearing perfume or cologne when visiting our office, because some patients have difficulty breathing when exposed to these strong odors.

SLEEP PATIENTS

Bring your medication or a complete list of what you are taking, including the strength and how you take it. If you have had a recent sleep study or breathing test, please bring a copy of the results with you or advise our office where and when you had the sleep study.

Bring any available prior chest X-rays or CAT scans of your chest needed to evaluate your problem. We also need a copy of the radiologist's report. If you have had a recent breathing test at your referring doctor's office, bring a copy of the test results with you. Bring either your medication or a complete list of what you are taking, including the strength and how you take it.

ADDITIONAL INFORMATION

When you visit, bring complete insurance information, including your insurance cards. Bring your referral forms from your primary doctor if you have an HMO or managed care plan. We'll call you a day or two prior to your visit to confirm your appointment. If you cannot keep this appointment, please notify us as soon as possible to reschedule.
